Weekly habit tracker
Habits as rows, days as columns, eight rows by default. Weekly rather than monthly, because a monthly grid is where habit tracking usually goes to die.

Weekly beats monthly
A monthly habit grid is thirty-one columns of small boxes, and the two things that go wrong with it are predictable: the boxes are too small to mark comfortably, and a gap in week one sits there judging you for the remaining three weeks.
A weekly sheet resets. A bad week ends on Sunday, which is both more honest and more sustainable than a month-long record of one difficult fortnight.
Eight habits, and why not more
Published templates run from five to twelve. Eight is the default here because it is enough to cover the things that actually repeat without becoming a second to-do list.
The rows are blank. Filling all eight on day one is the most common way to abandon a tracker; three or four that you genuinely intend to do is a better start, and there is no penalty for the empty rows.
